- Eligibility screening and application assistance for MediCal and Healthy Families insurance
- Referral and follow-up services for health care specialists such as cardiology, orthopedics, etc.
- Assistance in accessing mental health and other health services (labs, pharmacy)
- Case advocacy to assist families with basic needs (food, clothing, shelter) and to reduce barriers to health care access (transportation, language, information, etc.)
- Outreach/information sharing at community events, health fairs, and inter-agency meetings
- Active participation in collaborative efforts to improve health care delivery systems
- Sliding scale services for uninsured children
- Assistance with some costs of follow-up care
